Fayetteville (WSYR-TV) – Police have identified a man who was killed in a dump truck incident at a quarry in Fayetteville on Thursday.

Police say the driver – 49-year-old Terry Johnson of 108 Pleasant St., Manlius – was killed when he and a passenger leapt or fell from the moving dump truck at the Kinsella Quarry.

They say their initial investigation indicates that a possible mechanical failure compelled the two people to jump out of the moving vehicle. The passenger suffered only minor injuries.

Police are still investigating the incident, along with OSHA and the Mine Safety and Health Administration.
