Fayetteville (WSYR-TV) - Brake failure is to blame in a fatal crash in Fayetteville last week.

The Mine Safety and Health Administration says the brakes on 49-year-old Terry Johnson's dump truck stopped working when he was on Kinsella Quarry June 21.

Johnson and an unidentified passenger jumped from the moving vehicle to escape, but it ran over Johnson, who died.

The passenger suffered minor injuries.
